ABSTRACT
El síndrome de pinzamiento costoclavicular es una complicación infrecuente en los pacientes con dispositivos venosos centrales insertados por vía subclavia. Se presenta por la compresión externa del catéter en el ángulo costoclavicular, que puede generar obstrucción, ruptura y embolización de su fragmento distal. Usualmente se asocia a signos clínicos y/o radiológicos premonitorios. Dadas sus graves consecuencias, se debe hacer una adecuada profilaxis desde el momento de la inserción del catéter hasta el final del tratamiento. Se presenta el caso de un paciente con adenocarcinoma de colon, quien, durante la quimioterapia, desarrolló el síndrome de pinzamiento costoclavicular con ruptura y embolización del fragmento distal del catéter a la arteria pulmonar derecha, el cual se trató sin complicaciones mediante cateterismo por vía femoral.
Pinch-off syndrome is a rare complication of central venous catheter insertion through the subclavian approach. It is secondary to external compression of the catheter in the costoclavicular angle, which can lead to obstruction, rupture and embolization of its distal fragment. It is usually associated with premonitory radiological and clinical signs. Due to the severe complications of this syndrome, a proper prophylaxis should be done from the moment of insertion until the end of treatment. We present the case of a patient undergoing chemotherapy for adenocarcinoma of the colon, who developed pinch-off syndrome, with rupture and embolization of the distal fragment of the catheter to the right pulmonary artery. Treatment was successful by means of catheterization through the femoral vein.
